Dubious wording

The present article states that 'At that time [i.e. when the original organisation was founded in the 1950s] the rules of childbirth dictated that women did as their doctors told them'. What are these 'rules of childbirth'? Were they written down anywhere? Who enforced them? And which doctors are they talking about?
